29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about January 7, 2022, and at times prior thereto, employees were performing service and maintenance tasks on the Carpet Cushion Department Laminator without each applying a lock to isolate the energy sources, exposing the employees to the hazard of being caught-in moving parts.
