29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: A) Centrex Plastics, LLC, doing business as American Plastics, located at 814 W Lima St, Findlay, OH 45840: On or about May 17, 2020, the employer did not ensure that employees were adequately trained in electrical safety-related work practices such as, but not limited to, the use of special precautionary techniques and personal protective equipment. Employees were exposed to electrical hazards of 480 volts while conducting activities such as troubleshooting, cleaning, repairing, and replacing electrical components such as, but not limited to, wires for the heaters of the Toshiba Plastic Injection Molding Press # 51 (Model: ISF720SI-59A; Serial # 415705).
