29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: (a) Ranch Foods Direct LLC at 1228 East Fillmore Street Colorado Springs, CO 80907: On or before 6/11/21 the employer did not perform a workplace hazard assessment to determine which hazards existed, how they would be controlled and what personal protective equipment would be assigned to each task. This condition exposed employees to hazards including but not limited to amputation hazards and chemical hazards. Abatement Note: Abatement certification is required for this item.
