29 CFR 1910.132(d)(2):The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person which certified that the evaluation had been performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment: a) Jobsite located at 5000 S. Broad St. Philadelphia, PA - 19112: The employer did not conduct a workplace hazard assessment, properly identifying the hazard(s) associated with specific processes and adequate personal protective equipment (PPE) for employee use; thereby, exposing employees to hazards such as electrical, etc. An employee suffered severe injuries after coming in contact with an energized (480V) exposed conductor while attempting to obtain voltage readings. Noted on or about March 4, 2021.
