29 CFR 1910.184(i)(9): Removal from service. Synthetic web slings shall be immediately removed from service if any of the following conditions are present: 29 CFR 1910.184(i)(9)(iii) Snags, punctures, tears or cuts; 29 CFR 1910.184(i)(9)(iv) Broken or worn stitches: Northeast, saw department, single shipping and receiving dock, Demag 10 Ton serial no. 95433 overhead bridge crane, type VI sling 8-foot long nylon return/reverse eye: On or about June 21, 2017, the employer provided a sling with snags, punctures, tears, cuts, and broken/worn stitches to unload delivered steel bundles while hand-guiding basket-hitched loads to a temporary staging location. Exposed employees to crushing hazards associated with sling failure. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
