Doren, Inc. dba Omni Precast., on or about October 19th, 2021 and times prior thereto: An in-patient hospitalization was not reported to OSHA within 24 hours. The accident, and resulting hospitalization, took place on or about October 19th, 2021. The incident was not reported to OSHA until October 22nd, 2021.
