29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) 11045 Old Hwy 31, Spanish Fort, AL: On August 20, 2020, an employee was crushed by the slide of a Slide Rail System, being installed in a trench/excavation at River Oaks Phase I, South Hickory Street, Foley Alabama on August 20, 2020 resulting in multiple fractures. The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours, as required by the standard.
