29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling: Instances including, but not limited to, the following: a) Job site, 607 Penn Avenue, Pittsburgh, PA: On or about October 30, 2017, during work activities, the employer did not ensure that equipment was used in accordance with listing and labeling instructions in that a portable space heater was plugged into a relocatable power tap, which is not permitted by UL listing, and is specifically prohibited in UL 1363, standard for relocatable power taps.
