29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: (a) On or about January 10, 2022 - New 4x6 machine, employees were exposed to caught-in hazards when the machine specific procedure did not specifically describe the step(s) to verify lockout effectiveness. The procedure said the following: Try equipment for operation and return to neutral or off position.
