29 CFR 1904.40(a):The employer did not provide an authorized government representative the records within the four business hours. (a) 319 Lockheed Street, Pueblo CO 81001: On or about January 8, 2021, the employer, Key Structures, LLC, did not provide OSHA with the requested OSHA 300, 300A, and 301 forms or equivalent within 4 business hours. The employer did not provide OSHA form 301 or equivalent for an employee who was injured on September 9, 2021 due to a fall from a scaffold. (b) 319 Lockheed Street, Pueblo CO 81001: On or about January 8, 2021, the employer, Key Structures, LLC, did not provide OSHA with the requested OSHA 300, 300A, and 301 forms or equivalent within 4 business hours. The employer did not provide OSHA form 301 or equivalent for an employee who was injured on January 6, 2021 due to a fall from a ladder.
