29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log. On or about November 15, 2019, at the company establishment located at 15865 Assembly Loop in Jupiter, FL 33478, the employer did not correctly fill out an OSHA Form 301 for Case#11 for calendar year 2019, which occurred on August 26, 2019, was not accurately filled out and/or was missing information.
