29 CFR 1904.39(a): The employer did not report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident within 8 hours: a) Galen Drive Side of Building - The employer did not report a work-related death within eight (8) hours to OSHA after the death of the employee, on or about July 4th, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
