29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: (a) Summit Point Raceway, Summit Point, WV: On or about June 7, 2014 the employer failed to report the fatality of a part time driving instructor that suffered fatal injuries during a driving accident while riding with a student driver during the Hyperfest event. ABATEMENT CERTIFICATION REQUIRED.
