29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: NAS North Island: On or about October 3, 2013, the employer did not report a workplace death of an employee to OSHA within eight hours of the fatality becoming known to the employer. The fatality was reported to OSHA October 4, 2013, approximately 30 hours after the employer became aware of the death of its employee.
