29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) At the main office, 118 Doy Lanier Road, Statesboro, GA, 30458, on or about August 4, 2022, the employer did not record the following workplace injuries or illness on the OSHA 300 or equivalent for the calendar year 2022. On April 25, 2022, an employee suffered an intra-articular fracture of the distal radius extending into the radial styloid from falling off of a ladder while performing work on an agricultural pivot.
