On or about July 8, 2022, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2022. (a) Bleach Plant Operator suffered illness related to chlorine dioxide exposure that occurred on or about June 13, 2022 resulting in the employee's death on June 27, 2022.
