29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about November 2, 2021 at the worksite located at Upper Bass Lake Road, north of Zoar, Wisconsin; the employer did not report a work-related fatality within eight hours of being notified. The employer was notified of the fatality at approximately 5:00 pm on November 2, 2021 and did not contact the Appleton Area Office until November 3, 2021 at 8:49 am.
