29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): a.) 2K Wholesale, Inc., on or about May 26th, 2021 and times prior thereto: Company employees that were operating a Toyota brand forklift (model #8FGU30) (Serial #14506) were not certified by the company to operate the forklift onsite. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
