29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a.) Bessemer Concrete, LLC. - An in-patient hospitalization occurred on or about September 15, 2020 due to an accident at the address of 205 West Washington Street, New Castle, PA. The hospitalization was not reported to OSHA until October 1, 2020.
