29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employees amputation or an employees loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) This violation occurred on or about September 18, 2020, at the work place located at 4117 Pinnacle Point Drive, Dallas, TX, when the employer did not report to OSHA a right index finger amputation from a work-related event that occurred on September 18, 2020.
