29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300: A.) Microtech Knives, Inc., 300 Chestnut Street Ext., Bradford, PA 16701, on or about November 22, 2019 and times prior thereto: Column F of case #7 on the 2019 OSHA Form 300 included the description of the injury and part of the body affected, but did not include information on the object/substance that directly injured the employee. B.) Microtech Knives, Inc., 300 Chestnut Street Ext., Bradford, PA 16701, on or about November 15, 2019 and times prior thereto: Column F of case #6 on the 2019 OSHA Form 300 included the description of the injury and part of the body affected, but did not include information on the object/substance that directly injured the employee. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
