29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Industrial Hanger & Conveyor Co., on or about October 16th, 2019, at the location of 886 County Rd 232, Fremont, OH 43420: A work-related incident resulted in an employee suffering an amputation on or about the morning of October 15th, 2019 that was not reported to OSHA within 24 hours. OSHA was first notified by the employer on or about October 21st, 2019.
