29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On May 15, 2019, an employee transferring chemicals from a liquid tender truck was injured when an electrical arc between an electrical power line, Terra-Gator applicator machine and the employee standing on the ground suffered electrical shock which resulted in the in-patient hospitalization and multiple amputations. The hospitalization nor amputations were reported within 24-hours to the local Area Office of the Occupational Safety and Health Administration. The Peoria Area Office was notified on May 21, 2019.
