29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Holtville Gin Co-Op, 260 Lightwood Road Deatsville AL; On or about February 14, 2018 the employer had an employee suffer the amputation of a finger. The employer failed to report the work-related amputation to OSHA within twenty-four (24) hours of December 18, 2017 when the amputation occurred.
