29 CFR 1904.39(a)(1): The employer did not report within 8 hours the death of an employee resulting from a work-related incident. On or about August 28, 2015, an employee was asphyxiated while working in a confined space containing hazardous vapors. The employer did not report the fatality until September 2, 2015 which was more than 8 hours after the fatality occurred.
