Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling from ladder of moving crane(s), i.e., employee(s) were observed riding on the ladder giving access to operator's cab of moving Travelift Crane(s). (a)Gulf Coast Pre-stress, Inc., North Market Street, Pass Christian, MS - On or about Wednesday, July 31, 1996, at 1:32 P.M., while MI-JACK Series 750-D Travelift (Straddle Carrier) Crane #7 traveled in a northerly direction, from the area east of Pouring Beds #0-#3 toward the North Product Storage Yard, an employee was observed standing on the ladder and holding onto the crane operator's cab riding with the moving crane. (b)Gulf Coast Pre-stress, Inc., North Market Street, Pass Christian, MS - On or about Tuesday, July 30, 1996, while MI-JACK Series 750-D Travelift (Straddle Carrier) Crane #4 traveled in a northerly direction from the Pouring Bed area toward the Barge Loading Pier, an employee rode on the crane-cab-access-ladder to retrieve twenty (20) 4 x 4 timbers to use as cribbing at South Product Storage Yard. (c)Gulf Coast Pre-stress, Inc., North Market Street, Pass Christian, MS - Throughout the production and product storage areas of this facility riggers are allowed to ride on cab-access ladders for their respective Travelift Cranes. Among other methods, one feasible and acceptable method to abate this hazard would be to enforce manufacturer's recommendations prohibiting the practice of allowing employees to ride ladders.
