29 CFR 1904.39(a): The employer did not orally report the death of an employee within eight (8) hours after death of any employee from a work-related incident or the in-patient hospitalization of three or more employees as a result of a work-related incident. On or about September 15, 2014, the employer had an explosion that resulted in two employees sustaining sever burns, the employer failed to notify the OSHA Area Office in Fort Worth, Texas of a fatality that occurred on September 29, 2014 that resulted from the incident.
