Section 5(a0(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazards associated with unsafe operation of pressure vessels: a) Rolling Division 1) 45" Mill, Oil and Pump Cellar - The oil separation surge tank was not provided with a pressue gauge on or in the near proximity of the vessel to determine its operating pressure. An intervening stop valve was installed between the pressure relief device and vessel creating a potential hazard in the event the valve was in the closed position, observed August 7, 1989. 2) 40" Mill, North End of Oil Cellar - Two oil surge tanks with an operating pressure of 30 p.s.i.g. nominal, were not provided with pressure relief devices and the nameplates on each vessel were not legible, observed August 10, 1989. 3) 80" Mill, Coiler Area - The coiler oil surge tank with an operating pressure observed at 22 p.s.i.g. nominal, had two intervening stop valves installed between the vessel and the pressure relief devices creating a potential hazard in the event the valves were in the closed position. The system's pressure relief devices that were located on the two oil pumps were not marked or tagged with the required data, observed August 28, 1989. Among other methods, one feasible and acceptable abatement method would be to recognized and meet Commonwealth of Pennsylvania Act No. 451 (Boiler Law) and regulations governing unfired pressure vessels.
