29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite: 2250 E Cambria St, Philadelphia, PA 19134 - On or about August 17, 2024, the employer did not train employees conducting a brick fa�ade operation from the 5th and 6th levels of a supported tubular welded frame scaffold, who were exposed to fall hazards of up to approximately 33' 2", to recognize the hazards associated with working from scaffolds and to understand the procedures to control or minimize those hazards.
