29 CFR 1910.146(c)(4): The employer did not develop and implement a written permit space program that complies with this section. The written program was not available for inspection by employees and their authorized representatives: Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to protect employees from confined space hazards while performing maintenance, Clearing a jam or cleaning machines. On or about 10/26/2021, the employer failed to develop and implement a written permit space program that complies with 1910.146. The written program was not available for inspection by employees and their authorized representatives. Employees were entering into permit-required confined spaces on machines such as, but not limited to the following: a) The large "Harris Selco" baler (Model: HLO-8221AR-150; Serial No: 39770079) on the paper side of the plant; b) The small "Harris Selco" baler (Model:L125-2-10/8 ; Serial No:08138825 ) on the paper side of the plant; c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101) on the paper side of the plant; and, d) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658) on the plastic side of the plant.
