29 CFR 1926.1431(d)(5)(v): Anti-two block device was removed during personnel lifting operations: (a) Boom Truck Crane - On or about November 17, 2015 a Monitex 22101S boom truck crane used to hoist employees in a personnel platform did not have an anti-two block device connected to the boom. The boom crane was mounted on 2003 Sterling flatbed truck.
