29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Outside of electrical shed: On or about 12/19/2019, employees were exposed to electrical hazards from electrical that was not installed and used in accordance instructions included in the listing and labeling. 1. A metal outlet box that was not being used in accordance with its listing and labeling and was outside in wet conditions, was not rated for outdoor or wet locations. 2. A metal outlet box was used to make an extension cord for temporary wiring. 3. Circuit breaker panel did not have the correct manufacturer's breakers installed, leaving a gap between the breakers and inner cover that would affect the arc flash/blast rating. 4. A transformer in the electrical shed was not secured to the floor or by other appropriate means.
