29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: On or about April 26, 2023: Employees engaged in window replacement and upgrade on a residential home located at 2536 South 191st Circle, Omaha, Nebraska, were exposed to multiple fall hazards in that the employees were using a fiberglass extension ladder on the top level of the scaffolding as means of increasing the working level height. This created an unstable working surface and as a result the ladder and scaffolding collapsed and the exposed employee fell approximately 20 feet to the ground below whereby suffering fatal injuries.
