29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: On or about November 15, 2022 and at times prior thereto, in the Assembly area: a. Extension cord used to power the clip/U-Haul machine had damaged insulation, exposing employees to electrical shock and burns hazards. b. Extension cord used to power the Air King Fan had damaged insulation, exposing employees to electrical shock and burns hazards.
