29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work. Location: a) Supported scaffold on the north side of the building: On or about September 20, 2022, employees installed a winch hoist, for material hoisting, on the top tier platform of the scaffold. Subsequently, the employees removed two planks from the scaffold platform underneath the hoist, to provide an opening to hoist material. The employees that installed the hoist and removed the planks were not trained in scaffold erection, moving, dismantling or alteration.
