29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: At the worksite located at Route 2, Box 475, Willow Springs, Missouri: Employees were exposed to amputation hazards when cleaning out saw dust from the edger. Employees did not de-energize the edger prior to servicing the edger.
