29 CFR 1910.269(w)(7): Backfeed. When there is a possibility of voltage backfeed from sources of cogeneration or from the secondary system: On or about November 12, 2023, employees of Transformer Power Services LLC were performing testing on an electrical transformer located at 15141 State Highway 177 in Jackson, Missouri. The employer did not ensure the proper wiring of a Caterpillar generator into the transformer control cabinet. The improper wiring of this energy source provided a backfeed through the auxillary transformer energizing the arrestors and arrestor cable of the transformer. An employee exiting the transformer roof touched the Y1 tertiary bushing and was electrocuted.
