29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) AMG Construction, LLC, on or about July 1, 2019 at the address of 4087 Fleetfoot Rd. Coldwater, OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, guardrail systems or alternative measures while installing top plates to the top of a residential structure. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of up to 16ft.
