29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks: a) Lots 505/506 - an operator was not present at the controls of a Caterpillar TH460B telehandler while employees worked from an elevated platform which was attached to the boom approximately 27-feet high above grade, on or about March 17, 2023. b) Lots 505/506 - Employees utilized an elevated platform which was connected to the boom of a Caterpillar TH460B telehandler at a height of approximately 27-feet above grade, and the employer failed to perform an inspection of the equipment after making a change in the attachments, on or about March 17, 2023. c) Lots 501 through 506 - The employer allowed employees to operate a Caterpillar TH460B telehandler with a cracked windshield obstructing the view of the operator, on or about March 17, 2023.
