29 CFR 1910.269(o)(3)(i): The employer did not provide safeguarding within test areas to control access to test equipment or to apparatus under test that could become energized as part of the testing by either direct or inductive coupling and to prevent accidental employee contact with energized parts: On or about November 12, 2023, employees of Transformer Power Services LLC were commissioning and testing a 161Kv electrical transformer located at 15141 State Highway 177 in Jackson, Missouri. The employer did not provide safeguarding to employees when wiring a Caterpillar diesel generator into the control cabinet of the electrical transformer, under test. The 240v used to power the transformer control cabinet also provided a path to energize the auxiliary transformer. The arrestor cable and arrestors on top of the transformer became energized exposing employees to electrocution hazards.
