29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: FEDERAL CORRECTIONAL INSTITUTION - PHOENIX: The Internal Revenue Service failed to ensure that agents handling firearms treated each weapon as a loaded weapon, exposing employees to struck-by hazards from incidental shootings. On August 17, 2023, an IRS Special Agent was fatally shot during a routine training exercise.
