29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy for energy isolation and control: The employer failed to provide training to every employee authorized to perform lockout at the facility. Employees who service and maintain equipment such as the shavings bin that was equipped with an auger were exposed to caught in/caught by, entanglement and amputation hazards from rotating parts. On or about November 22, 2021, an untrained employee (Millwright by trade) was tasked with fabricating a cover for the shavings bin and auger which remained operational at the time of the incident. The employee was inadvertently pulled into the machinery resulting in a fatal incident.
