29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. A) C.H.A.T of Pennsylvania, On or about May 05, 2021 at the address of 5650 Wattsburg Rd., Erie, PA: A Hyster forklift was being used with a man-lift attachment. The attachment was used for activities including, but not limited to, lifting employees for rack teardown. The company had not received the forklift manufacture's approval to use this attachment.
