29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in Sec. 1910.6: On or about October 7, 2019, at the LPG dispensing station, liquefied petroleum gas fuel was not stored or handled in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases, NFPA No. 58-1969: A) The employer did not ensure employees filling LPG cylinders were qualified to the standards required by NFPA No. 58-1969 Section 1.5, which included, but were not limited to, training employees on proper LPG handling procedures at least once every three years, and ensuring employees understand emergency response procedures. B) The employer did not ensure at least one qualified person remained in attendance at the transfer operation from the time connections were made until the transfer was completed, shutoff valves were closed, and lines were disconnected, in accordance with NFPA No. 58-1969 Section 4.2.1.1. C) The employer did not ensure LPG cylinder storage was located to minimize physical damage or tampering, in accordance with NFPA No. 58-1969 Section 5.2.1.1. D) The employer did not ensure LPG cylinder storage was located at least five (5) feet away from the dispensing station, in accordance with NFPA No. 58-1969 Section 5.4.1.3. E) The employer did not ensure LPG cylinders in storage were protected against vehicle impact provided in accordance with good engineering practices where vehicle traffic normally was expected at the dispensing station, in accordance with NFPA No. 58-1969 Section 5.4.2.2.
