29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At a residential worksite located at 2826 SE 8th St, Blue Springs, MO: Employees were exposed to caught-in and struck-by hazards. This was demonstrated when an employee was working inside an excavation that was longer than 25 feet and was only provided a 6 foot tall ladder as a means of egress in an excavation that was 12.3 feet in depth.
