29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about January 4, 2018, the employer did not ensure employees followed lockout/tagout procedures during the service and maintenance of a Resnor 2.5 million BTU Industrial Furnace. The following was not completed: The furnace was not shut down [per the 1910.147(d)(1) requirements], all energy sources, to include Natural Gas, were not isolated [(d)(3)] , lockout/tagout devices were not applied to each energy source to include Natural Gas [(d)(4)], and it was not verified that the equipment energy was isolated and de-energized prior to starting work [(d)(6)].
