29 CFR 1910.217(c)(2)(i)(d): Fasteners not readily removable by the operator, so as to minimize the possibility of misuse or removal of essential parts, were not utilized on point of operation guards. On or about July 16, 2021, an employee operating the Roper Whitney Pexto Press to make filter straps received an amputation and the front metal fabricated guard was affixed to the machine with a pair of vice grips which did not minimize the possibility of misuse or removal of the guard.
