29 CFR 1926.1204(b): The employer did not identify and evaluate the hazards of permit spaces before employees entered them: A) 133 East Clarke Place, Bronx - NY: An elevator mechanic and a mechanic?s helper entered the elevator pit when the elevator car was at an upper floor. The employer did not identify and evaluate the physical hazard in the elevator pit, on or about February 18th, 2021. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
