29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a) Cooper Tire & Rubber Co., North Air Dryer, - On or about November 4, 2020, employees installing a valve on the intake header of the air dryer did not place locks on the air dryer energy isolating devices, exposing employees to a struck-by hazard.
