29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. Location: Jobsite On or about October 28 2019, an employee was injured from a fall of 16 feet to the ground below while working where the fall arrest system was not capable of preventing contact with the next lower level or limiting the free fall to not more than six feet. Actual accident. October 28, 2019, Employee fell 16 feet and made contact with ground below.
